Acute & Chronic Urticaria
Distinguishing a short-lived reaction from a chronic pattern shapes everything about how urticaria is managed.
Book a Hives EvaluationSudden onset of raised, itchy welts?
A single episode, especially with an identifiable trigger, is likely acute urticaria and often resolves with simple treatment.
Hives recurring for six weeks or more?
This is classified as chronic urticaria and typically needs a more structured, sustained management approach.
What Is It?
Urticaria, commonly known as hives, is classified as acute (lasting under six weeks, often linked to an infection, food, or medication) or chronic (lasting six weeks or more, frequently without an identifiable external trigger) — and this distinction meaningfully changes both the evaluation and treatment approach.
Acute urticaria is common and often resolves either on its own or with straightforward antihistamine treatment. Chronic urticaria is a different picture — often without a clear external cause, sometimes linked to underlying autoimmune activity, and generally requiring a more structured, sustained management approach, including antihistamine doses higher than standard over-the-counter recommendations in many cases.
Risk Factors
- Acute: recent infection, new food, or new medication
- Chronic: often no identifiable external trigger, sometimes linked to underlying autoimmune activity
- Physical triggers: pressure, cold, heat, or exercise in some patients
Symptoms
- Raised, itchy welts that move around and resolve within 24 hours at any given spot, even as new ones appear elsewhere
- Acute: lasting under six weeks
- Chronic: lasting six weeks or more, often with a fluctuating course
Treatment
- Antihistamine Therapy: First-line treatment for both acute and chronic urticaria, sometimes requiring higher-than-standard doses for chronic cases under medical guidance.
- Escalated Therapy for Chronic Cases: For chronic urticaria not controlled with antihistamines alone, additional treatment options considered based on severity.
