Fillers vs. Botox: Which One Actually Addresses Your Concern?

Botox and dermal fillers are often mentioned in the same breath, but they work in fundamentally different ways and address different concerns. Knowing the distinction helps you have a more useful conversation at your consultation.
Botox Relaxes; Fillers Add Volume
Botox works by temporarily relaxing the muscles responsible for expression lines — frown lines, forehead lines, crow’s feet. It doesn’t add anything to the skin; it reduces the muscle movement that creates certain wrinkles over time.
Fillers work the opposite way — they add volume directly to areas that have lost it, such as the cheeks or under-eyes, or to enhance contour, as with lip enhancement. They don’t affect muscle movement at all.
Matching Treatment to Concern
If your main concern is expression lines that appear when you smile, frown, or raise your eyebrows, Botox is the more directly relevant treatment. If your concern is more about hollowing, lost volume, or wanting more definition in a specific area, fillers are the better fit. Many patients benefit from a combination, addressing both kinds of concerns where both are present.
Why This Distinction Matters for Expectations
Using the wrong treatment for a given concern doesn’t just waste money — it can also mean the actual issue goes unaddressed. Under-eye hollowing, for instance, isn’t something Botox meaningfully improves, and expression lines caused by muscle movement won’t be softened by adding volume alone.
A Conservative Approach to Both
For both treatments, especially in areas like the lips and under-eyes where overcorrection is most visible, we favor a conservative starting approach and build gradually based on how the initial treatment looks, rather than aiming for a dramatic change in a single visit.
